Notes/Domino 5.0.8.

Excerpt from the Lotus Notes and Domino Release 5.0.8 MR fix list (available at
http://www.lotus.com/ldd):

SMTP Server
SPR# EJIL4GXHL5 - Added Personal Stationery form to the mailbox.ntf template,
from mail50.ntf. This regression was introduced in 5.0.

NOTE: Ensure the setting "Format for messages addressed to internet addresses"
in the client Location document is set to "Notes Rich Text Format". Recycling
the Notes Client may be necessary. If the header image shows as an attachment
to the recipient, enable the thumbnail view in Windows Explorer for the folder
in which the image resides.
