This issue was reported to Lotus software Quality Engineering and has been
addressed in Domino 5.0.12, Domino 6.0.4 and Domino 6.5.2.

Design - Design Task
SPR# OGRL57QF3J - Fixed a problem where the inbox was created twice when the
design task ran on a newly created database.
It is very important that the design task not be run until all the databases
have been initialized.
Note: You may need to create agents to fix this problem if you have many mail
databases with two Inboxes. More >
