Problem:
This issue occurs only with OLE 1 type objects.
This issue was reported to Lotus Quality Engineering, and has been addressed in
Notes 5.0.6 Client and Domino 5.0.6 Designer.
Excerpt from the Lotus Notes and Lotus Domino Release 5.0.6 QMR fix list:

SPR# MGAN4MZTJJ - Fixed a problem where OLE 1 objects were not saved after edit.
Workaround:
If possible, obtain a later version of the OLE application which creates OLE 2
type objects. Using this application create a new OLE object in the Notes form
and then copy the information from the older OLE object into the new one.Supporting Information:
