SPR# MBOK8T7TH2 - Fix to Convert -m which was not removing all folder references. The fix adds a scan of the database after the views are scanned, for any folder ref items that were missed and removes the items from these notes also. New ini variable DEBUG_DISABLE_CONVERT_FOLDER_REFS_REMOVE_ALL=1 can be set to disable the new code.
