Getting alot of CLFAD0211E: Exception thrown which crash the HTTP task

This is an app that is being rolled into production after almost a year of development and testing.
 
It uses several extension library functions. The server is running the 29 June release (I think).
 
It uses the XPages error page.
 
The CLFAD0211E: Exception thrown is usually followed almost immeadiatly by: "Out of memory exception occurred servicing request for: /Apps/CIS/PFSClientInfoSystem.nsf/tsPeople.xsp - HTTP Code: 500".
 
The only way to recover, once this occurs is to stop and start the HTTP task. Three times yesterday it caused the entire domino server to crash and restart.
 
We have increased theHTTPJVMMaxHeapSize from 64m to 128M and seen no change in the time before the failure occurs.
 
Load on this app is about 150 people at peak.
 
Anyone else seen this type of crash?
