IMSMO and multiplied mail

Does anyone know more about the bug which has this comment in the 2.0.1.4 notes about IMSMO "Add protection around reported bugs where outgoing email can multiply in Outbox and spam recipients"? Is the bug solved or what does "Add protection around" mean. I had a user today that experienced multiplied mail (1000+) on version 2.0.1.6.
