Authentication Failure, no update to inetlockout.nsf

I just noticed that I am receiving authentication failure messages on my console for users attempting to activate Traveler devices:

     07/18/2013 03:16:51 PM nHTTP: user.name [255.255.255.255] authentication failure using internet password

While this is all well and good, my "inetlockout.nsf" database is not being updated, as it had been in the past.  I haven't changed anything on the server, and the Config doc has the security set to lockout a user after 10 wrong passwords.

Any ideas on what could have changed such that users dont' get locked out now?
