"User name is not unique" in a Delivery Failure Report

I have been having a strange issue the past week. I have staff and faculty users emailing students, who are located in a secondary addressbook. When they email the students, they staff members will get a delivery failure saying the name isn't unique. The name is in fact unique. I have those users open up the delivery failure email and click the resend button and then click send and the email goes through without any problems.

Any ideas on how to even troubleshoot this issue. It doesn't seems to happen all the time for every user, but enough users that it's becoming a big issue.

Thanks for any suggestions.

I should also mention that we are using Directory Assistance and a condensed Directory Catalog (Dircat) in our environment.
