How To Example: Web SELECT (not XPages)

Here's an example and how to for anyone that wants a Notes based but BootStrap / JQuery style web based select field.  I put it into a .nsf so you can test it out without any configuring.  Instructions on the About db document.  Please post questions below if you have any issues/questions.
