Creating a partially completed email form?

I have a situation where I'd like to create a partially-filled memo, grabbing some field values from a doc and letting the user add comments with each one. I can use formula language to create a blank email or to build and send a complete email, but I don't believe I can partially fill in the body - just the To and Subject for example. I can use LotusScript to build and send a more complex email, but again I don't think I can go halfway.

If I can't go halfway, I see two alternatives:
1) Build a note and send it back to user to add comments to and forward to the intended end user. This isn't perfect but it would work if they uphold their end and follow through.
2) Prompt for an input when they send the note and adding that text to it as an overall remark. I'm not sure this will suffice because I'd rather spread the comments among the fields I'm embedding, but it's worth looking into.

Am I missing something or are those my choices (without doing something extremely complex)? Here's an example of what I'd like:

To: Reviewer
From: Doc Author
Subject: Please review this

Field 1 value: xxxxxxxxx
Rationale: (user-filled)

Field 2: yyyyyyy
Rationale: (user-filled)
...
Field n: zzzzzzzx
Rationale: (user-filled)

Link back to doc the fields came from --> [ ]

Why not...

Just create the document using a form with all the fields and comments fields, let the user add their comments, then email the completed document?
