Problems with client connection documents?

Has anyone else had problems with connection documents being ignored by the Notes client?  I have a number of 9.0.x users (myself includes) who have connection documents in their PNAB alonf the lines of server/Org -> server.orgdomain.com but these seem to be completely ignored.

Do a file:open application... and entering the FQDN in the server field fixes it for the current session AND puts a new connection document in place in the form described above if there isn't on there already, but the next Notes restart seems to discount them completely and the user has to this again to 'remind' the client how to get to the server (even though the connection document is still there).

I'm sure this used to work fine in 8.5

Some thoughts about it
If the workstation was upgraded from 8.5 to 9, maybe some processes that use to take place on server weren't run on it, so:
- some internval views may not be correctly indexed and need to be rebuilt
- design of local names.nsf may not have been updated correctly
- local cache may not have been updated or be corrupted

In either case, I would suggest:
- replace design of names.nsf to ensure "Personal Address Book" (pernames.ntf) R9 template is applied to it
- enter the database and press Ctr+Shift+F9 to reindex all views
- doubleclick a workspage page tab, reduce the size assigned to design elements cache to a minimum and compact it:
       
- Close Notes
- delete cache.ndk
- Restart Notes client

Then try to open a database on a server you were not able to before this procedure.
