Empty menu bar on XPages rich text control

On one of my newly upgraded servers the Rich text control menubar is empty. I have several other servers (both fresh install and upgrades) without this issue.

 

Also the rich text editor is much smaller than on the other installations. 

 

Any idea on where to start working on this error?

Solution - deleted browser cache.

Embarrasingly enough the problem was resolved by deleting browser cache. Thanks to IBM support.
