So you mean it is no problem to refresh the design of the database when it is opened - if you just close it and reopen it?

Andreas,  You need to make sure the db is closed and clicked away from on the Workspace or you may not get a complete refresh/replace.  I usually initiate the refresh by doing a right-mouse click over the db (db...\Refresh/Replace design), and then once it has started, I click away from it.  That way there is no longer any "hook" into the db and it does a clean refresh/replace.
