Attachment Icon Distorted

Hi, In my Notes Client based application, my users attach multiple spreadsheets, etc. inside rich text fields in a Notes document.  Over time as the document is saved and re-saved, the shape of the icon of the attachments become stretched or elongated.  In the same database, in other documents, a shadow or phantom icon has appeared beside the real icon. The shadow cannot be opened and has no properties. Examples below.

Any ideas what is causing this or how to prevent?  Thanks!

Earlier discussion on same issue...
Been discussed before, didn't look like anyone had a solution.  I would open a PMR with IBM.
