Contacted IBM support, the iNotes "Forms" database was causing the problem. We renamed the old iNotes\forms85_x.nsf and re-created it with a replica from a server where iNotes is working fine, restarted HTTP and now it works.
Just learned a new troubleshooting tip for iNotes: try this type of URL for the user in question, if we suspect a given user is having iNotes trouble, it forces the mail db to open using vanilla HTML look and feel: http://servname/mail/usersmaildb.nsf?opendatabase&ui=webmail
