'optimize document table map'

Do you have the property 'optimize document table map' enabled for this database?

If so, you might try disabling it and see if it helps. You may have to run compact on the database after disabling the property.

The statement you made re: Shift-F9 resolving the issue made me think of this. We've fought this issue literally for years on a handful of databases that has a high-turnover of documents. It appears to be fixed as of R9, ODS52.

Of course, this may not be it at all.
