Question

You say you have a mix of single / multi server sso. Are you sharing the same ltpa config name?

Is there a pattern where the user accesses a single server sso and then hops over to a multi server sso or visa versa and the ltpa key in the browser is confused?

Are you sure that the name showing in the domlog is resolvable in the name.nsf ($users) view.
