Forms disappearing from database

Recently, I have had forms disappearing from my database design.

For some reason, the forms that have disappeared are forms I use as dialogboxes. In one database, on two separate occasions, two different forms have disappeared. The forms are in the template and I simply refresh the design and the forms are put back in the design. But I don't know why the forms disappeared in the first place.

In two other databases, the exact same thing has happened. And strange that in both databases, it was a form that I use as a dialogbox that disappeard.

Any ideas? I mean, I simply refresh the design and they come back.. but wonder why they are being removed.

My administrator said, the forms must have become corrupted and get removed when the server runs fixup on them. Just weird that this is happening all of the sudden. I think in the past month, I have had 4 forms disappear from various databases.
