Ok I'll chekc for replicas

The dbs do get updated from a template. The template had the missing forms in it, so I just refreshed and the missing form came back.

But I'll check to see if there is a replica copy that for some reason doesn't have that form. What is weird is I have not made any changes to this db in about 8-9 months, and now all of the sudden forms are disappearing.
