I have had great success, we have about 250 users and we are now blocking 700-900 messages a day. I know that my users have seen a difference and are very happy, you don't get that reaction very often. As far as false positives, I am really not sure if we have gotten any yet, no reports so far. I have also set it so that the rejected user would get a message say which site blocked them. I have had the best results so far with sbl.spamhaus.org leading the number and the next best was bl.spamcop.net. The rest have a small number of hit in comparison.
I would say dive in, the rewards are better then the risks. Best of luck.
