This is purely a guess, but based on the information you've provided, its shouting "backlogged 3rd party server virus scanner queue" to me.

Virus scanners will only bother to queue up for inspection those emails containing attachments.

Its also possible that the user is using a remote "Island" or "Home" Location document set to use Local mail, and that they switched to an "Office" location at 11am. That's the moment when the mail would actually leave their local machine's local outgoing MAIL.BOX.
