I have another possible reason a mail file does not get upgraded. I don't want to mess my system up by verifying it, but I had the policy document working successfully to upgrade several mail files I used during testing. Then, today, two or three users upgraded to 6.5 and the mail file was not upgraded, and no email notification to say it failed.
I had recently removed myself from the Policy Creator/Polciy Modifier roles in the address book (there was a reason). Since I had created the setting and policy document, I think that I had to have those roles set in order for the policy to be enforced.
